This is a Family Practice Clinic. We provide comprehensive primary medical care from new born to elderly. We focus on preventative health care with a patient centered approach as a Team.
Please book an appointment ONLINE if possible through the Website (https://omamedical.cortico.ca/book/family-doctor/) or over the PHONE (604 -873-6624). We are UNABLE to book appointments if requested through emails.
Our email address is only to be used to send us information we have requested. Please put “Dr. Jayaraman REQUESTED this information” in the subject line and the email will be forwarded to Dr. Jayaraman. All other emails will be returned unread due to the large number of emails we are receiving requesting things of the staff. Any requests for filling out forms, medication refill, reports etc need to be made through an appointment.
